Program Info
Eligibility
Frail seniors or adults (age 18 or over) with a disability residing in the city of Oakland or Alameda. Serve frail adults who have the following conditions: high blood pressure, diabetes, stroke, cardiovascular conditions, dementia, and any other condition that causes difficulty with living independently.
Fees
No fee for Medi-Cal eligible participants and a sliding scale fee for private-pay individuals.
