Program Info
Eligibility
Low-income adults living with cancer who are currently in active treatment and living in Contra Costa and Alameda County. Income eligibility: $30,000 a year or less ($2,500 x mo). Must be referred by patient navigator or social worker.

How to Apply
A medical professional on your care team (social worker or nurse navigator) will need to apply on your behalf. They can call the Operations Manager for the application at (510) 809-0217.
