Eligibility
Open to California residents with:. Chronic conditions. Gestational diabetes and other high-risk pregnancy conditions. Chronic or disabling mental or behavioral health disorders. Patients discharging from the hospital or skilled nursing facility. High risk of hospitalization or nursing home placement. Care needs that require multiple doctors
