Visit during program hours, call for an appointment. Accept referrals from other agencies and self referrals. All referrals should begin with a call or email to set up an appointment. Can not accommodate drop-in referrals.
Documents Required
ID, insurance verification, proof of residence in State and County, proof of HIV status, proof of income, and contact information for physician or clinic where receiving care. Any additional information you can provide is always appreciated.
