Individuals unable to provide for their basic needs (food, clothing, shelter) or manage their own financial resources (including ability to resist fraud or undue influence) due to mental disability, poor health, dementia or other disabling condition. Requires statement signed by a physician confirming qualifying disability.To qualify for a Probate Conservator under the Public Guardian's Office, there must be assets. Assets are other than monthly income that could not otherwise be managed by a non-court appointed representative payee. Requires statement signed by a physician confirming qualifying disability.

Fees vary as ordered by Superior Court.

Call for application to be mailed. Referral may be made by hospital administrator, social worker, or any knowledgeable relative or friend who can provide the facts which may warrant a conservatorship. Hospital referral necessary from treating psychiatrist for conservator under the LPS/Probate Conservatorship Laws.
