Open to seniors who are 65 or older, frail or disabled who:Live in Oakland/Emeryville/Piedmont or Berkeley zip codes 94704 or 94705Receive monthly Medi-Cal benefitsAre at risk of nursing home placement because of frailty or inability to manage without helpAre willing and able to accept care management
