Description

*Individuals who may not qualify (older former foster youth) can still be referred under a INFORMATION REFERRAL as they can receive information or attend workshops that could be helpful for their circumstances.*Provides housing navigation and intensive 1-on-1 support for Non-Minor Dependents (NDMs) to connect to transitional housing, build housing readiness, and prepare for stable independent living. Services Provided:Housing Navigation SupportProvides information and connection to transitional housing programs and housing options youth may qualify forHousing Readiness Assessment Assesses youth for readiness and need related to housing placement Housing PlanningSupport youth with developing a Housing Plan for transitional housing placement and/or independent living Intensive 1-on-1 Coaching Provides individualized support to prepare youth for independent living and housing stabilityPROGRAMS GOALS / OUTCOMESIncrease Youth knowledge of transitional housing resources Improve Youth housing readiness and ability to follow a housing planIncrease youth preparedness for independent living Support in maintaining stable housing and stability

Office is open for in-person support 9am - 5pm Monday-Thursday. Staff of the Day are available over the phone 9am-5pm on Fridays as office is closed .
