Program Info
Eligibility
Resident of Alameda County; income below 200% of Federal Poverty Level; and no health coverage and/or health coverage does not cover service needed. HealthPAC eligibility is approved for up to one year, and must be renewed each year by updating enrollment information.
Fees
No fee for enrollment. Individuals may need to pay a co-payment for services depending on the service and their personal situation.

Must show proof of identity, residency and income sources. If applicant is a U.S. citizen or National and has 5 years of Legal Residency, should also be able to show proof of citizenship.
