Open to residents of Alameda County, who are. Age 19 or over, and. Have limited income (income at/below 200% ofthe Federal Poverty Level), and. Are not eligible for public health insurance, and. Complete the HealthPAC application

Must show proof of identity, residency and income sources. If applicant is a U.S. citizen or National and has 5 years of Legal Residency, should also be able to show proof of citizenship.
