Description

Offers intensive community support service and an Assertive Community Treatment for individuals with severe mental illness. Serves individuals who are high utilizers of mental health services. Clients must be considered to be at great risk for hospitalization. Staff provides a full range of services, including: medical services psychiatric services case management services advocacy and linkage referral to safe and affordable housing substance use interventions and counseling assistance with entitlements support and education with family and significant others connection with community resources, self-help groupsPlease note: Offers ongoing services during Public Safety Power Shutoffs (PSPS) and Disasters. Services are available if power is turned off in this facility.

Eligibility

Adults ages 18 and older.Resident of Alameda County.Clinical determination that the person is unlikely to survive safely in the community without supervision.Person's condition is substantially deteriorating.Person has been offered chance to participate in development of their treatment plan for services and continues to fail to become involved.Have a history of lack of participation in a mental health program that have resulted in 2+ hospitalizations or incarcerations within the last 36 months, or have attempted to cause harm in the last 48 months.
