How to Apply
Call for an appointment. Accept referrals from other agencies and self referrals. All referrals should begin with a call or email to set up an appointment. Can not accommodate drop-in referrals.
Documents Required
ID, Medi-Cal verification, proof of residence in County (Alameda or San Francisco), proof of HIV status, proof of income, and contact information for your primary care physician or clinic where you receive care. Any additional information you can provide is always appreciated.
