Eligibility
Must self -identify as an individual with a chronic, serious, or advanced illness or a caregiver of someone with a chronic, serious, or advanced illness and provide consent to participate. Unable to serve those with cognitive impairment, but can work with their caregivers. Individuals MUST be able to give written or verbal consent to participate in the program and a resident of Alameda County
