CEI Support Services

Description

Offers people 55 and older, dealing with multiple health challenges and who need help in their daily routine to stay in their homes. These services include: medical care transportation activities meals medications PT/OT/rehab dietitian dentist optometry podiatry home care Please note: Offers services during Public Safety Power Shutoffs (PSPS) and Disasters. While facilities will close during a power shut-off, any needed transportation will be provided and additional home visits may be made to check on participant safety.

Program information

Eligibility
Resident of service area. Adult, age 55 or over, must meet Medi-Cal requirements for nursing home level of care, be able to live safely in the community.
Program Fee
Medi-Cal, Medicare accepted. Medi-Cal and Medicare eligible receive the services free unless there is a Medi-Cal share of cost.
Application Process
Call for intake. Appointment necessary for center tour.
